TURKISH (STICKER) TOURIST VISA REQUIREMENTS

1. Duly completed Application Form,You may fill up the attached draft form
2. 2pcs photograph. (Photo size should be about 2 x 2 inches (5 x 5 cm) square with white background. Do not staple or glue the photo to the application form. Please make sure it is not damaged)
3. Original Passport – valid for 6 months
4. Old Passport showing history of travel
5. If employed:

• • Certificate of Employment including applicant’s position, working record and monthly/annual income;
  • Approved Leave Certificate indicating the travel date
  • Individual Latest Income Tax Return

6. If Self-employed / Business owner:

• • Copy of the Company Business Registration such as SEC and DTI,
  • Copy of Mayor’s Permit
  • Copy of Business ITR

7. Original Bank Certificate addressed to the Embassy and Bank Statement covering the last 6months transaction
8. PSA issued Birth Certificate
9. PSA issued Marriage Contract
10. Hotel Accommodation Booking
11. Itinerary Flight Details
12. Travel Insurance
13. Special Power of Attorney – Must be filled out and notarized. This serves as your authorization letter

Visa fee: USD $290 per applicant – Payable to Travel Specialist Ventures
